ELAHERE Support Services Case Managers are specialists who can support you during your treatment journey. Once you are enrolled in the ELAHERE Support Services Program, your Case Manager will help you understand your insurance coverage and navigate financial assistance options.
How can ELAHERE Support Services help me with the cost of my ELAHERE prescription?
Once you are enrolled in the ELAHERE Support Services Program, your Case Manager will help you understand your insurance coverage and navigate financial assistance options.
- If you have commercial insurance and are eligible, you may pay as little as $0 per dose for ELAHERE with the co-pay assistance program*
- If you are uninsured, underinsured, or have Medicare or Medicaid, you may qualify to receive ELAHERE at no charge through the Patient Assistance Program (PAP)†

*Terms and conditions apply. Patients are eligible for co-pay assistance if enrolled in private commercial health insurance and are not covered by state or federal healthcare programs, and who meet the eligibility criteria. Patients will be enrolled for 12 months. There are no income requirements to participate in the program.
†Criteria include: patients who are uninsured or have insurance that excludes coverage for ELAHERE (including patients on Medicare or Medicaid), residents of the United States or Puerto Rico, and patients who meet the financial eligibility requirements. Terms and conditions apply.

- Will I need to see other specialized doctors?
- What can I do to get ready for treatment?

Question for your eye doctor:
My oncologist put me on a drug that can affect my eyes. Would your office be able to work with my oncologist to help monitor my eye health during my treatment?
